Shift Pursuit has an exciting opportunity available at a private hospital located in the East Rand, Gauteng for a skilled Clinical Pharmacist with a strong focus on antimicrobial stewardship and patient-centred care. This role is centred around delivering advanced clinical pharmacy services within a ward-based environment, ensuring optimal medication use, patient safety, and improved clinical outcomes.
Requirements:
Pharm. Qualification
Master’s degree in clinical pharmacy or equivalent / higher as assessed by Council.
Registered with South African Pharmacy Council
Comprehensive knowledge of the requirements of the relevant legislation
Antimicrobial Stewardship course(s), Infection Prevention and Control related course(s)
Minimum of three to five years’ professional experience as Clinical Pharmacist
Experience in managing an Antimicrobial Stewardship Program
Knowledge of recent clinical trials
Computer skills (Excel, Word, PowerPoint, etc.)
Key Performance Areas:
Clinical pharmacy services
Focus on achieving pharmacovigilance within the ward-based setting, optimising the use of medications, emphasising dosing, monitoring, identification of adverse effects, and economic efficiency to achieve optimal patient outcomes.
Ensure the provision and monitoring of customer focused, professional clinical pharmacy services.
Develop policies and procedures specifically for clinical pharmacy and hospital medication management.
Enhance therapeutic drug monitoring and ensure effective medication reconciliations are performed.
Take pharmaceutical leadership role in clinical protocol and guideline development.
Professional collaboration
Development of pharmaceutical guidelines and protocols
Act as a leading pharmaceutical partner to enhance cooperative and influential working relationships with doctors, microbiologists and other key functional stakeholders.
Provide advanced clinical services to medical specialties.
Provide drug information and training to other professionals as required.
Collaborate with nursing and infection prevention.
Provide mentoring and work-based support to pharmacists to develop skills, capacity and professional competency within the pharmacy.
Antimicrobial stewardship
Elevate the hospital’s performance on antimicrobial stewardship.
Monitor microorganisms cultured and compile antibiograms.
Identify inappropriate utilization of antibiotics by collecting and analysing data.
Implement restriction policies for specific antibiotics to prevent misuse or overuse.
Patient medication safety
Lead clinical audits of medication
Elevate medication safety management and interventions of medication adverse events.
Implement drug administration protocols.
Relationships and professional profile visibility
Perform or contribute to research, teach and publish in clinical pharmacy.
Participate in internal or external stakeholder meetings and industry led conferences.
